Position Summary:
At Athletico, the Physical Therapist’s role is to evaluate, design and implement a patient’s plan of care. Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.

 

Essential Duties and Responsibilities:

The below is not an exhaustive list of duties and you will be expected to perform different tasks as necessitated by your changing role within the organization and the overall business objectives of the organization.

  • Assess and screen patient's daily living/work‐related skills and develop therapeutic retraining programs with measurable  
  • Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient’s level of function.
  • Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care
  • Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range‐of‐motion and function or teach compensation
  • Review/expand the treatment programs in a manner consistent with the functional goals of each patient and within the guidelines of the rehabilitation program 
  • Report patient status and progress to the physician, rehabilitation staff, patient and family me
  • Participate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's
  • Recommend and/or design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and/or community
  • Train family members and caregivers to carry out the treatment including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings
  • Accurately and thoroughly document patients' care observations, interventions and evaluation on the day services are Ensure that interim (verbal) orders received from the physician are accurately documented and implemented. Submit evaluation, treatment plans and discharge summary within twenty four (24) hours.

Qualifications:

  • Education:
    • Degree from an accredited Physical Therapy Program

  • Work Experience:
    • A minimum of 5 year physical therapy experience preferred
    • Home Health experience preferred

  • Certification/Licensure:
    • Current Physical Therapist licensure in good standing

  • Knowledge and Technical Skills:
    • Current CPR certification
    • Proof of a negative TB test recommended
    • Exceptional organizational and communication skills
    • Current and valid driver’s license
    • Current and valid automobile insurance

Language Skills:

  • Ability to read, write and speak English proficiently

  • Specific vision requirements include the ability to see at close range, distance vision, peripheral vision, depth perception, and the ability to adjust focus
  • While performing the duties of this job, the employee is regularly required to speak and understand
  • Possess the ability to fulfill any office activities normally expected in an office setting, to include, but not limited to: remaining seated for periods of time to perform computer based work, participating in filing activity, lifting and carrying office supplies (paper reams, mail, etc.)
  • Frequently required to stand, walk, sit, use hands to feel, and reach with hand and arms
  • Occasionally lift and/or move up to 20‐25 pounds
  • Fine hand manipulation (keyboarding)

Work Environment: 

  • Internal office and patient home
  • The noise level in the work environment is usually low
